What is the Medicare GLP-1 Bridge?

It's the first time Medicare has covered GLP-1 medications for weight loss — a short-term CMS program running July 1, 2026 through December 31, 2027 that covers brand-name Wegovy, Zepbound (KwikPen), and Foundayo for a low fixed copay, for people who meet published criteria.

Why "Bridge"?

Medicare law has long excluded weight-loss drugs from Part D coverage. Rather than wait for that to change, CMS built a temporary program that runs alongside Part D — a bridge between the old rule and whatever comes next. It uses its own central processor for prior authorizations and pharmacy claims, which is why prescriptions need special routing instructions (and why doctors who haven't seen the program yet appreciate a heads-up).

Who it's designed for

Adults 18 and older with Medicare drug coverage who want a GLP-1 specifically for weight reduction or maintenance, and who met one of three BMI pathways when starting therapy: BMI 35+, BMI 30+ with certain conditions, or BMI 27+ with certain risk conditions. Notably, people who've been paying out of pocket — through LillyDirect, NovoCare, or compounding pharmacies — are squarely in the program's intended audience. Self-pay history doesn't disqualify you; only receiving a GLP-1 through your Part D plan routes you elsewhere.

What it isn't

It's not a coupon, not an enrollment site, and not automatic. Your own clinician decides whether a GLP-1 is medically appropriate, writes the prescription with the program's routing instructions, and completes a prior authorization.
