Criteria updates
The Bridge is a new program and CMS can change its rules. Every screener result and packet shows the criteria version it was built with, and any change we make is recorded here — so you always know whether your packet reflects the current rules.

Based on published CMS materials, including the prescriber fact sheet "Medicare GLP-1 Bridge: Information for Prescribers" (CMS Product No. 12235, June 2026). Covers the three BMI pathways, the Part D routing exclusions (type 2 diabetes, moderate-to-severe sleep apnea, MASH, prior plan-paid GLP-1), the self-pay clarification, plan-type rules, and covered drugs and formulations (Wegovy injection/tablets, Zepbound KwikPen only, Foundayo).
